17-04-12 Player Registration Form 2012-13
The team/player registration form for 2012-13 has been sent to each club secretary.
The form can also be downloaded from here..... ( download form ).
To make your life easier, there's no need to supply the following details (unless different/changed from last season):
Club : Address, Secretary
Player : Address, Postcode, Tel, Mobile, D.O.B, IM number
So, basically, all I require is : Club Name, Team no., Home Night, Start Time, Pref/Div, Player Name(s)
